أَحْمَدُ بْنُ مُحَمَّدِ بْنِ عِيسَى عَنْ عَلِيِّ بْنِ أَحْمَدَ عَنْ مُوسَى بْنِ عُمَرَ عَنْ مُحَمَّدِ بْنِ مَنْصُورٍ قَالَ: سَأَلْتُهُ عَنِ اَلَّذِي يَنْسَى اَلسَّجْدَةَ اَلثَّانِيَةَ مِنَ اَلرَّكْعَةِ اَلثَّانِيَةِ أَوْ شَكَّ فِيهَا فَقَالَ« إِذَا خِفْتَ أَنْ لاَ تَكُونَ وَضَعْتَ وَجْهَكَ إِلاَّ مَرَّةً وَاحِدَةً فَإِذَا سَلَّمْتَ سَجَدْتَ سَجْدَةً وَاحِدَةً وَ تَضَعُ وَجْهَكَ مَرَّةً وَاحِدَةً وَ لَيْسَ عَلَيْكَ سَهْوٌ»
Ahmad ibn Muhammad ibn Isa, from Ali ibn Ahmad, from Musa ibn Umar, from Muhammad ibn Mansur, who said:
I asked Him عليه السلام about one who forgets the second prostration of the second rak‘ah or doubts about it. Imam عليه السلام said: "If you fear that you may have only placed your face once (in prostration), then when you finish the prayer with the salam, perform one prostration, and place your face once. And there is no (obligation of) prostration for forgetfulness (sahw) upon you."
